Background Osteosarcoma is a most common, highly malignant bone tumor affecting children and adolescents in their early ages but it may occur in late ages after an initial bone disease such as Paget Disease . There are two types of osteosarcoma primary osteosarcoma Secondary osteosarcoma Telangiectatic osteosarcoma. It is  least common variant of osteosarcoma. Total hip replacement (non cemented)

35 yrs old male presented with pain and limping gait for the last 2 years. On x rays we diagosed him as a case of a vascular necrosis of rt head With no history of diabetes, hakeem medication, sex medication, trauma and negative family history. Base line and special investigations are normal. We planned after medical fitness, cardiac evaluation and anaesthesia assesment thr of rt hip. Pre op Per op Post op Unevent ful recovery We kept patient on partial weight bearing for 6 weeks
